The Case: Early Pregnancy with Unexpected Findings
A 29-year-old client presented to her midwife for a routine early pregnancy visit at approximately seven weeks based on her last menstrual period (LMP). She reported mild nausea but no pain or bleeding. Her history was unremarkable—no prior complications or medical conditions.
As part of her comprehensive assessment, the midwife performed a limited obstetric ultrasound to confirm an intrauterine pregnancy and estimate gestational age. The client was excited to see her baby for the first time.
However, the scan revealed an empty gestational sac measuring 20 mm with no visible yolk sac or fetal pole—findings inconsistent with the reported gestational age.
Rather than offering assumptions, the midwife explained the possible scenarios clearly:
- The pregnancy could be earlier than expected due to late ovulation.
- The findings could indicate an anembryonic pregnancy (a type of early pregnancy loss).
With empathy and professionalism, the midwife discussed next steps and offered a follow-up ultrasound in 7–10 days for clarification.
